Admitted to 12th Field Ambulance, 11 August 1918, and transferred to 1st Casualty Clearing Station (rheumatism); discharged to unit, 18 August 1916.

Taken ill in the field, 5 September 1916; admitted to 1st Casualty Clearing Station, 6 September 1916 (ingrown toenail, varicose veins); to Hospital Train, 9 September 1916; admitted to 16th General Hospital, Wimereux, 9 September 1916; discharged to duty, 11 September 1916; marched into 5th Divisional Base Depot, Etaples, 13 September 1916.
