Military Medal 'On 29th September, 1918, at BONY. He was one of a party detailed to search for Booby traps and road mines on forward roads, the party closely following the attacking Infantry. The attack was held up at GUILLEMONT Farm and severe casualties incurred, and the attackers disorganised. Corporal HAZLETT set a splendid example of coolness and resourcefulness in assisting to reorganise the line, which done, he patrolled the road and helped in the dismantling of several suspiciously wired mines in dugouts under the roads.'
