Admitted to 1st Field Ambulance, 29 June 1916 (scabies); transferred to 1st Australian Casualty Clearing Station, 30 June 1916; transferred same day to 59th Casualty Clearing Station; condition diagnosed as venereal disease, 3 July 1916; transferred to Ambulance Train, 4 July 1916, and admitted to 18th General Hospital, Camiers, 5 July 1916; discharged to Base Details, 10 October 1916; total period of treatment for venereal disease: 101 days.

Admitted to 18th General Hospital, Camiers, 17 October 1916 (venereal disease: gonorrhoea: slight); transferred to 51st General Hospital, Etaples, 23 November 1916; dischaged to Base Details, 5 December 1916; total period of treatment for venereal disease: 50 days.

Found guilty, 6 January 1917, of when on Active Service (1) breaking out of hospital (2) absent without leave from Tattoo, 31 December 1916, till 10am, 5 January 1917: awarded 28 days' Field Punishment No 1, and forfeited 6 days' pay.

Accidentally killed by explosion whilst salvaging ammunition, 12 April 1917.

Court of Enquiry, Bazentin, 17 April 1917, concluded that the casualty occurred through the accidental explosion of a 4.7" shell; further that every possible care was taken in handling same, and that no blame rests on anyone concerned.
