Admitted to 1st australian dermatological hospital, Bulford, 18 July 1916; discharged, 8 August 1916; total period of treatment for venereal disease: 22 days. Admitted to 1ADH, 18 August 1916; discharged, 17 October 1916; total period of treatment for venereal disease (gonorrhoea): 61 days.

Found guilty, 28 October 1916, of being absent without leave, 17 October to 4 pm, 19 October 1916: awarded 5 days confined to camp and forfeited 3 days' pay.

Marched in to 4th Training Bn from No 1 Command Depot, Perham Downs, 2 November 1916.
