'Essequibo' 16 January 1917 and embarked for England ex Alexandria. Arrived in England for Imperial Service in France 26 January 1917; left England for duty in France 8 February 1917. Attached to 11th British Stationary Hospital, Rouen, 11 February 1917. Died en-route to 8th General Hospital, Rouen, of cerebro-spinal meningitis.
