Admitted to 3rd Australian Field Ambulance, 22 January 1917 (scabies); transferred to 36th Casualty Clearing Station, 24 January 1917 (pyrexia, unknown origin: serious); to Ambulance Train No 22, 25 January 1917, and admitted to 12th General Hospital, Rouen, 26 January 1917; transferred to England, 9 February 1917 [date on form B103 is, in error, 9/8/17] (trench feet), and admitted to 2nd Birmingham War Hospital, 10 February 1917; transferred to 3rd Australian Auxiliary Hospital, Dartford, 2 April 1917; discharged on furlough, 5 April 1917, to report to No 1 Command Depot, Perham Downs, 21 April 1917.
