Inscription on headstone, as requested by Mrs Mary Ann Taylor (mother), Coolgardie Road, Southern Cross, Western Australia: 'Our Tom is sleeping his last long sleep. In a grave we may never see. Some gentle hands in that far distant land may plant a small flower for me. R.I.P.'

Distinguished Conduct Medal

'For conspicuous gallantry and devotion to duty. When all the officers of his company had become casualties, during a counter attack on strong enemy positions, this N.C.O. organized two platoons under heavy fire, and continued the advance. When the final objective was reached,Corporal Sullivan set about the work of defence and consolidation with great skill and initiative, exposing himself in the execution of his scheme with complete disregard of his own safety. Throughout the operations he displayed great gallantry and ready resource.'
