Meritorious Service Medal

'This N.C.O. has carried out the duties of Sergeant Cook since March, 1916 in a highly efficient and reliable manner. During long periods in the forward area under adverse conditions and trying circumstances his work was of the utmost benefit to the Battalion, and it is mainly due to this N.C.O.'s careful supervision and untiring efforts in the feeding of the men that the Battalion's record for few evacuations had stood out well in the Division. He has many times been under heavy hostile shelling and has once been wounded. His zeal, conscientiousness, ability and loyalty are valuable assets to the Battalion.'
